The reimbursement of travel expenses is only up to the limit specified for each recipient.
Please follow the instructions listed below:
- You will need to fill out an IEEE expense request form. This form should be filled out and accompanied with all original receipts (the IEEE will not accept copies). In order to facilitate processing, it would be appreciated if receipts are taped to blank sheets of paper and not stapled. In order to complete the form you must include your name, mailing address, name of the conference attended, the name of the Technical Committee granting the award, and the maximum amount of the award. You must list each item you are claiming for reimbursement on the form and an accompanying receipt must also be attached.

Certificate of Foreign Status of Beneficial Owner for United States Tax Withholding (W-8)
or
Request for Taxpayer Identification Number and Certification (W-9)
(for U.S. citizens). - Mail all completed forms to:
